Helpful Information for New Patients

We want to make your first visit to Family Medical Care as stress-free as possible.  So here are a few things to keep in mind for your first visit.

1. If you are a new patient, you will need to fill out a few brief forms upon arrival.

2. Please bring your insurance card, driver's license, a picture ID, and a method of payment. Credit card, cash, debit cards and personal checks are also accepted.

3. If you need to change or cancel an appointment, please call us in advance if possible.

4. If you are taking any of the following tests, you will need to fast the night before the test is given: cholesterol, blood sugar level, and blood work for a physical.

We are also in the process of adding a new patient portal to our website, which will allow you to download important patient information.

You may visit our care center without an appointment.  But if you have a life-threatening emergency, please go to the nearest hospital Emergency Room for treatment.

We are open seven days a week.  While you may schedule a consultation, we gladly accept walk-in patients.  Visit either Bay Shore location or call the location nearest you to learn more about our services and payment arrangements.
